Reflection for parent teacher conferences

We are suposed to have a reflection for parent teacher conferences.  I am thinking the following components are important:

• grade in class
• accomplishments
• recognition of struggles or problems
• location of who they can get help from
• a plan to improve performance or maintain current level

In the interest of keeping prointing costs low, I am thinking that this is something that the student does on their own paper. 

 I am thinking about just using the overheads attached to do this.
